Step 1: Referral

We require a medical referral and medical records for all new patients.

Have your practitioner to send us a completed Referral Form to finish Step 1.
Step 2: Review

Our clinical team carefully reviews each new referral before making a decision.

This will take about 5 business days.

Step 3: Referral Decision
​

Appointments will only be offered if a referral request has been approved.

If you have not received a response after 5 business days please call our office.
Our office does accept most insurance plans.
